Subcontractor Qualification Form
New subcontractors are required to submit a
Qualification Form in order to work with
Freese Construction. The form will enter
your company into the database to be
contacted for projects bidding.

W-9 Form (Includes Instructions)
• Insurance Certificate – Must be an
original from the agent submitted on an
official ACORD certificate.
• Signed Contract – Must be fully executed
through project manager and is required prior to first payment.
• Signed Change Order – Must be fully
executed prior to being billed.
